Thus, he took the “chastisement”, or “punishment”, which our transgressions deserved. The word “pierced” interestingly occurs in Isaiah 51:9 of God’s smiting of the dragon and thus shows that the destruction of the powers of darkness was at the heart of the cross as God not only smites the devil but smites himself and removes the curse of sin. “He was pierced.” His wounds were necessary for our salvation. These blessings were to flow far beyond Israel and come to the whole world. ![]() “He” is emphatic-“it was he” or “he was the one”-there was no one else through whom all God’s blessings were to come. There have been multiple cases reported of seizures following the withdrawal of zolpidem. Suicide attempts and completion have been successfully linked with zolpidem use (OR 2.08 95% CI 1.83-2.63) in patients regardless of the presence of comorbid psychiatric illness. ![]() A systematic review of 24 previous studies of sleepwalking associated with zolpidem demonstrated that the association was not dependent on age, dose, medical history, or even a history of sleepwalking at any time before zolpidem use. A case series of 119 inpatients aged 50 or older demonstrated that a majority (80.8%) of ADRs were central nervous system (CNS)-related such as confusion, dizziness, and daytime sleepiness. The relative risk (RR) for hip fractures in patients taking zolpidem was described as 1.92 (95% CI 1.65-2.24 P<0.001), with hip fractures being the most commonly seen. Zolpidem has been associated with an increased risk of falls in hospitalized patients with an OR of 4.28 (P <0.001) when prescribed short-term for insomnia. However, zolpidem has a wide variety of adverse effects and has some special considerations noted in the literature. One of the most used of these hypnotics is zolpidem. Non-benzodiazepine hypnotics such as zolpidem, eszopiclone, zaleplon are the most used as adjunctive treatment. The most effective therapies utilize cognitive behavioral therapy in conjunction with pharmacotherapy to minimize the needed dose and any resulting side effects. The treatment of insomnia should involve a multi-disciplinary approach, focusing on implementing behavioral interventions, improving sleep hygiene, managing psychological stressors, hypnotic treatment, and pharmacological therapy. Insomnia can significantly impact daytime functioning resulting in decreased workplace productivity, proneness to errors and accidents, inability to concentrate, frequent daytime naps, and poor quality of life. The sleep disturbances in insomnia usually manifest as difficulty in falling asleep, maintaining the continuity of sleep, or waking up too early in the morning well before the desired time, irrespective of the adequate circumstances to sleep every night. It can be controlled, but proper treatment is key.Insomnia is a common type of sleep disorder defined by an ongoing difficulty initiating or maintaining sleep or nonrestorative sleep with subsequent daytime impairment. In the end, I was always able to receive help from fellow scribes and the medical staff whenever I had questions or needed assistance. I found meeting with physicians beforehand allowed me time to feel well prepared in advance of their patient visits. This can be difficult for new scribes, as you must learn to implement different ways of charting according to the provider’s preference and layout of each clinic. ![]() Furthermore, there are three different medical offices where our physicians practice, each with its own equipment, dedicated team of receptionists and medical assistants, and unique layout for exam rooms. Currently, I typically work with a single physician on a given day, although I may also work with more than one provider during busier periods.Īlthough we use one EMR system clinic-wide, each provider tends to have a distinct way of formatting their exam findings and assessments. Once I was on my own, I found there was a steep learning curve for the first few months, from adjusting to the pace of the clinic to understanding each provider’s unique style and approach to patient visits. She taught me the ins-and-outs of the electronic medical record (EMR) system used to access and update each patient’s chart. How long did it take to be trained?Īfter I was hired, I trained for about one month, then was paired with another scribe who had prior experience so I could shadow her twice a week in the clinic. I was looking for an opportunity to work closely alongside physicians and observe their clinical decision making firsthand, which is why it was such a good fit for me. He highly recommended the position as a great way to gain clinical experience before starting medical school. I found out about medical scribing from an alumni of my graduate program, George Squared, who worked as a scribe in the same ENT clinic where I’m currently employed. How did you find out about medical scribing? In general, a medical scribe helps physicians provide more effective and efficient care by saving them time by documenting findings from patient exams and assessments. On a typical day, each physician in the office sees 20 to 35 patients for a wide range of needs, as well as pre-op and post-op consultations. What does a medical scribe do?Īs a scribe in an Otolaryngology clinic (also called “ENT”, for “ear, nose and throat”), I’ve transcribed hundreds of physician-patient encounters, medical histories, and diagnostic test results while working closely to assist pediatric otolaryngologists in the delivery of patient care. The benefits of being a medical scribe include gaining more experience in a medically related environment, earning income to pay off loans, saving money for medical school, and shadowing physicians while working closely alongside them. The medical scribe profession is an emerging field that some medical school applicants pursue while in college or during a gap year. Now, during graduate school, she is working to expand her knowledge of human anatomy and physiology, while also working as a medical scribe in an ENT clinic and volunteering as an EMT in her hometown. During that time, she also served as a Red Cross volunteer at Walter Reed National Military Medical Center. Having grown up in a rural, medically underserved town in Virginia, Morgan appreciated the seminars and discussions focused on creating a more equitable healthcare system with improved access and delivery of medical services. While there, she participated in the NIH Academy where she learned more about the social determinants of health. All that’s missing are the machine guns and air-to-air missiles. They might hunt with almost no visible effort, or they may dive and invert and roll, convincing you they’re having fun while doing it. Mississippi Kites are like F-16s in the air they’re fast, acrobatic, and buoyant. But if you happen to own cicadas, grasshoppers, or dragonflies, I have very upsetting news. ![]()
